Access Care Human Services, LLC is a Certified Medicare skilled and non-skilled Home Health Care Agency. We are licensed and bonded in the state of Pennsylvania. We are dedicated to delivering quality care to our clients in the comfort and privacy of their homes. Home is designed to be an alternative to the rising cost of care usually provided in hospital settings. Our dedicated team at Access Care Human Services provides skilled in-home nursing services by making clinical assessments, monitoring vital signs, and assisting in providing comprehensive, in-depth treatments for our clients. Our professional and dedicated Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Therapists assist in developing a comprehensive care plan under the guidance of their physicians tailored toward our client’s needs and preferences. Our dedicated professional team members also assist our clients and family members in developing a plan of care that brings together all services required to assist our clients in reaching their optimum well-being. We provide services in the following counties: Bucks County, Chester County, Delaware County, Montgomery County, and Philadelphia Counties.
